The following are FAQ’s patients may have prior to their visit at OrthoMemphis:
Is there anything I need to do before I arrive at the clinic for my appointment?
Yes, please complete your pre-visit paperwork. You can download the paperwork by clicking here, or if you prefer, OrthoMemphis will mail it to you prior to your visit.
What should I bring with me to my appointment?
- Please bring all current insurance cards and referral information.
- Any medical records related to the treatment of your condition/illness.
- Any x-rays, MRI’s, CAT scans, or bone scans related to the treatment of your condition/illness.
- A list of current medications you are taking.
- A list of any medications to which you are allergic.
- Someone to assist you in completing paperwork, if you are unable to do so yourself.
- Your co-pay or form of payment for your visit. We accept personal check, cash, Visa, MasterCard, and Discover.
If I have a billing question prior to my visit, with whom should I speak?
- You may speak to one of orthomemphis’ billing associates by calling 901.259.7600.
- OrthoMemphis’ billing office is open Monday through Friday between 7:00am and 4:30pm.
